I've never seen one of these before so I don't know what vehicles it was originally included with. Very likely, it was all Chevrolet vehicles equipped with PVC at the beginning of the "PCV era".
The cleaning instruction are nostalgic for me. When I worked in a Chevron gas station during the latter part of my high school days, we used to disassemble and clean these valves as part of vehicle service (i.e. lube and oil change). This was about 1963-65. However, the valves that were designed for disassembly and cleaning disappeared not too long after that time and were replaced by the type of valve that cannot be disassembled. So, I would say the instruction card you show had to be from about the 1961 to 1965 period.
PVC came to California before the rest of the country. I believe it started in California in 1961 but didn't become nationwide until 1963.In Appreciation of John Hinckley- Top
