    Put some loctite on the threads then.

      Great idea! I happen to have some on the shelf. By the way, if I am reading the assembly instruction manual correctly, they should be tightened to between 45 and 55 pound foot of torque, right?

          Those bushings are normally a pretty heavy press on the outer ring. I would not expect the bushing to be coming out as in the photo unless the press was not adequate even with the bolt missing. If bushing is urethane, this is a poor choice as stated previously.
