It looks to me almost like a GM #3937771 which was used on some 68-70 Corvette big blocks. However, the 3937771 has the shorter shaft and was not being manufactured as of early 1967. So, this is the "small block version" of the 3937771. I don't know what the part number was but I don't think it was ever used on a Corvette.
It may be a GM #3883158 which was used on some trucks during the late 60's. It's also possible it's a clutch used on some non-Chevrolet model GM cars or, even, some non GM cars.In Appreciation of John Hinckley- Top

I would tend to agree with Joe.
My 1967 Chevrolet Parts catalogs (April & July 1967) list GM # 3883158 as the fan-clutch part number for 65-66 and 67 trucks and is described only as "(7 1/8" dia.)" . GM # 3883158 is not listed in my 1966 Chevrolet Parts Catalog (Oct. 1965).
As far as I know the 1963-1967 Corvettes were issued only the 6" diameter fan-clutches.
