odd shifting on 454 with auto transmission

Once in a while on acceleration, my auto transmission skips second gear entirely on the way from first to third. I can't seem to correlate this behavior to anything.

As far as I know, the transmission is original, and has never had any work except for required fluid maintenance.

What should I be looking at?

I'd inspect the modulator (and make sure it's not sticking).
