When was the "three" hole divider used, and when was the "two hole configuration used (production dates/early/late)?

Per "Corvette by the Numbers" by Alan Colvin the three hole was used from August 1965 until June 1967. The two hole was then used for the 1968 model year.
The open plenum was used on L-88 cars only for model years 1967 and 1968. There are apparenly quite a few of the L-88 open plenum intakes that did not start their lives that way. Supposedly you can tell if they have been cut by the angle of the plenum cutout.
