Several. Crack open a copy of the Paragon catalog and you should find they offer the service in addition to independent individuals.

The trick lies in being able to de-crimp the original mirror housing and size + edge bevel the replacement glass to fit inside the crimp radius and finally re-crimp the housing to hold the replacement glass.

You can actually do this yourself, but the learning curve is kind of steep. Expect to 'botch' the first 5-6 attempts you execute...
