Michael, Right, who knows what happened. All speculation on my part, but maybe this...
As I mentioned earlier the pedal assembly may have been put together off-line, so possible the cap was there prior to the line.
Edit... It has been researched by a member that John Hinckley in fact said the pedal bracket was pre-assembled before installation.

Yours is a unique situation. Maybe the cap assy was once on your pedal bracket from new and a prior owner removed just the cap and left the clamp and screw. Then maybe your restorer left it as he saw it, but then decided new looked better so he just used new parts.
Again, speculation, but yours appears to show evidence just like Dave"s and the one I worked on.
