Hi there, Has anyone ever seen this location for a 1969 headlight washer (left side) Or is it a botched retro-fit?

The one on the drivers side is from a late 70 through 71. Passenger side is correct for your car.

Most likely the left side was replaced with a salvaged later unit. Of course, that raises the question of why it was replaced. These bezels are not wear parts and were a zinc die casting for 1968-72. So, a minor discrepancy such as you have noted could trigger a new chapter in understanding the complete history of the car.
