Jack, Thanks for the docs. I have not seen this document before. I assume this was an "optional" dealer provided service and may have varied from dealer to dealer? Thanks again.

I honestly have no further info/knowledge/usage on this particular form... just something I came across recently.
Glad some people liked it... perhaps there are even more forms for different years. Letz see'em!

Here is my 65 sheet. Dealer used plate to emboss the numbers on mine. Date would have been mid Feb., 65.

In the late 70's and early 80's it was called a "pre- delivery inspection" and dealers were paid a small fee to perform one. I think the amount varied depending on how much was involved on different models. During that time period, we had to record axle and engine numbers on that report. The dealership I worked at also recorded key numbers. Back then, you could call up the dealership or come by and prove ownership and get a key for $1.00.
