All originals I have seen did not have the insert, and as mentioned, they do not seem to have provison for one. When we had the parts business, the reproductions had it. Dick asked the vendor why it was there and the answer was that the one he had been given to copy had it.
When we were working on the present 1968-69 JG, someone sent a photo of a knob with the insert and its GM box showing the part number. That is when we deduced (or should I say "assumed"?) that service ones had it. Joe may have supplied the reason if it was used across other GM lines.
I have never seen one with the set screw.
