Anyone know what years this fan blade was used and what hp car or cars 18" Diameter with blades 6" x 4" stamped FRONT and H has round mushroom head rivets.

I would say that your fan is GM # 3749576 since it has domed or mushroom head rivets. This was was used on 58-63 Chev. Pass cars w/AC, 61 Chev. w/348, and 61-63 Chev. Pass cars w/327, 409 engines. GM # 3749576 was replaced with GM #3789562 in July 1964 as per Chevrolet Parts History for service parts.
The 3789562 Hayes-Albion fan (18" dia., 5 blades, 4 flat rivets per blade) and the 3863137 Schwitzer fan (18" dia., 5 blades, stamped "912239" along with a date code) were used on 66-67 Corvettes w/AC and other Chevrolet models w/AC.
There is also a similar 18" 5-blade fan with 5 flat head rivets made by Hayes-Albion ("H" stamping). Logic tell me that this fan is probably an "early" version of the 3789562 fan used on 1964 and probably 1965 Chevrolet models w/AC, but that is just an educated guess. My 1964 & 1965 Chevrolet Assembly Manuals list fan # 3789562 for models w/AC.

Here are photos of an early NOS 3789562 fan (18" dia., 5 blades, 5 flat rivets/blade). I assume that Hayes-Albion went to the 4 rivets/blade version of the 3789562 fan (66-67 models) to cut costs. Noland Adam's 63-67 restoration book have a shop drawing of a 3789562 fan (4 rivets/blade) on page 118.
