the horn relay on the upper drivers inner fenderwell, the question is how is it mounted to the inner fender ? a stud or just a couple bolts ? or where there variations ?

The horn relay is mounted to a single stud. The stud is part of a plate assembly which attaches to the wheel well side of the fender well and is attached via rivets. There were no variations.In Appreciation of John Hinckley- Top

The relay is mounted to to inner fender by a threaded stud plate riveted to the fender. The plate is mounted from behind the inner fender with the stud protruding through. A nut and lock washer is used to fasten. Section 12, sheet A5 of the AIM shows this pretty well. Hope this helps.

Yes, it's the same GM #3918535 stud assembly as used for the applications which you described and several other applications, as well. One other that comes to mind is the heater box mounting. This part went through several "iterative" part numbers, the last being GM #10147970. This piece used a metric stud of a size close to the original 1/4". It's discontinued, too, though.
