I have seen two different types of front upper shock mount washers that go underneath the upper nut. One is similar to that used on the "a arms", and one that is smaller with a "hex" shaped indentation for the nut to sit in. I would like to know which is judged as correct.

I can't tell you which one is "judged as correct". However, I can tell you which one IS correct. The one that IS correct is the one with configuration similar to the a-frame bushing retainers. It's completely convex (or, concave, depending upon which side you look at). It's 1-29/32" OD, has a 13/32" hole in the center and original examples have an oiled or dry film surface. The part number is GM #3790342. It's still available from GM, too.
The "hex indent" type upper retainer is the kind that is supplied with Delco SERVICE shock absorbers as part of the hardware kit that comes in the box. Instructions also provided say to discard all existing hardware and replace with that which comes with the kit. Many folks do and that's how these "hex indent" retainers "find their way" to the top position on Corvette front shocks. They were NEVER there originally, though.In Appreciation of John Hinckley- Top
